Athletic Foundations Program - Spring Registration

To enroll your son in our program, please complete the information below and sign.

Program Schedule: The 2024 Middle School Athletic Foundations program started January 10, 2024 and will continue through May 2024. It includes 8 training sessions a month for one hour each. Please note, this is a continuous program that will run on a monthly basis. By signing up for the Spring block of training, your son will be enrolled automatically each month that training is offered until you notify us of his discontinuation of our program (see cancellation policy below).

Training Times: Wednesdays 7:00pm - 8:00pm and Saturdays 9:30am - 10:30am

Billing: After completing this form, you will be redirected to a secure link to pay your deposit of $280 that will also cover April training. You will then be automatically billed $280 per month starting May 1st. Please note, that until your deposit is paid, we cannot guarantee a spot in the program.

Cancellation Policy: If you no longer want to send your son to the Middle School Athletic Foundations Program, cancel anytime before the next payment is due. There are no refunds for unused or partial months of training. For more information, please read the missed practice policy in our waiver form.

I have been informed that a receipt for each payment will be provided to me and the charge will appear on my credit card statement. I agree that no prior-notification will be provided to me unless the date or amount changes, in which case I will receive notice from The OH Line Academy at least 10 days prior to the payment being collected. I understand that this authorization will remain in effect until I cancel it in writing, and agree to notify The OH Line Academy in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I acknowledge that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this Credit Card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this sign up form.
