BEASST Program - Spring Sign Up

To enroll your son in our program, please complete the information below and sign.

Program Schedule: The 2024 BEASST program started January 10, 2024 and will continue through May 2024. It includes 8 training sessions a month for 90 minutes each. Please note, this is a continuous program that will run on a monthly basis. By signing up for the Spring block of training, your son will be enrolled automatically each month that training is offered until you notify us of his discontinuation of our program (see cancellation policy below).

Training Times: Wednesdays 5:00pm - 6:30pm and Saturdays 8:00am - 9:30am

Billing: After completing this form, you will be redirected to a secure link to pay your tuition. Please note, that until your tuition is paid, we cannot guarantee a spot in the program. If, you are signing up during the middle of a month for training, please do not pay, as we will custom invoice you for a prorated amount which will cover the remainder of the month.

Cancellation Policy: If you no longer want to send your son to attend the program, cancel anytime before the next payment is due. There are no refunds for unused or partial months of training. For more information, please read the missed practice policy in our waiver form.

By clicking this box, I authorize The OH Line Academy to charge my credit card on the first day of the month for each month that my son remains in the BEASST program for the amount listed of $340. I have been informed that a receipt for each payment will be provided to me and the charge will appear on my credit card statement. I agree that no prior-notification will be provided to me unless the date or amount changes, in which case I will receive notice from The OH Line Academy at least 10 days prior to the payment being collected. I understand that this authorization will remain in effect until I cancel it in writing, and agree to notify The OH Line Academy in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I acknowledge that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this Credit Card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this sign up form.
