High School D-Line Skills - Spring Registration

Program Schedule: The program starts April 1, 2024 and will run through May 25, 2024. This 8-week program includes 12 training sessions per month (3 per week) for 75 minutes each.

Practice Location: Weather permitting, practices will be held at the practice fields nearest to Dublin City Preschool, 7030 Coffman Rd, Dublin, OH 43017.

Film Location: Film sessions will be held at our training facility located at 8500 Warner Rd Ste. 200, Plain City, OH 43344.

GROUP 1

Training Times:

Mondays 6:15-7:30pm

Wednesday 6:15-7:30pm

Fridays 6:15-7:30pm

GROUP 2

Training Times:

Tuesdays 5:00-6:15pm

Thursdays 5:00-6:15pm

Fridays 5:00-6:15pm

***Please note: In the event that the minimum group attendance is not met, we reserve the right to consolidate training groups to maximize the training opportunities for all of our athletes. You will be notified of any scheduling changes via email and text***

Billing: After completing this form, you will be redirected to a secure link to pay your deposit of $400 that will also cover April training. You will then be automatically billed $400 for May's month of training on May 1st. Please note, that until your deposit is paid, we cannot guarantee a spot in the program.

Cancellation Policy: If you no longer want to send your son to this program, please notify us anytime before the next payment is due. There will be no refunds for unused, partial months of training.

I agree that no prior-notification will be provided to me unless the date or amount changes, in which case I will receive notice from The OH Line Academy at least 10 days prior to the payment being collected. I understand that this authorization will remain in effect until I cancel it in writing, and agree to notify The OH Line Academy in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I acknowledge that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law.
