Athletic Foundations Program - POST-SEASON Registration

To enroll your son in our program, please complete the information below and sign.

Program Schedule: The 2024 Post-Season Athletic Foundations program begins November 6, 2024 and continues through December 21, 2024 (with a break for the week of Thanksgiving). It includes 12 training sessions for 90 minutes each.

Training Times: Wednesdays 6:30pm - 7:30pm and Saturdays 9:30am - 10:30am

Training Dates: 11/6, 11/9. 11/13, 11/16, 11/20, 11/23, 12/4, 12/7, 12/11, 12/14, 12/18, 12/21

Billing: After completing this form, you will be redirected to a secure link to pay your tuition of $450. Please note, that until your deposit is paid, we cannot guarantee a spot in the program.

Cancellation Policy: If you no longer want to send your son to the Middle School Athletic Foundations Program, cancel anytime before the next payment is due. There are no refunds for unused or partial months of training. For more information, please read the missed practice policy in our waiver form.

I agree that no prior-notification will be provided to me unless the date or amount changes, in which case I will receive notice from The OH Line Academy at least 10 days prior to the payment being collected. I understand that this authorization will remain in effect until I cancel it in writing, and agree to notify The OH Line Academy in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I acknowledge that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law.
